Job Overview Join our dedicated healthcare team as a Home Health Aide (HHA), where your compassionate care and attention to detail will make a meaningful difference in the lives of our clients. In this vital role, you will provide personalized assistance in patients' homes, supporting their health, safety, and well-being. Your enthusiasm and professionalism will help foster a comfortable, respectful environment for individuals of all ages and needs, including those with developmental disabilities, dementia, or recovering from illness. This position offers an engaging opportunity to deliver high-quality home health services while working closely with healthcare professionals to promote independence and dignity. Duties Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using tools like Hoyer lifts or other assistive devices Monitor and record vital signs including blood pressure, pulse, temperature, and respiration to track patient health status accurately Administer medications and manage medical documentation in compliance with HIPAA regulations and EMR/EHR systems Support patients with specialized care needs such as Alzheimer's care, dementia care, hospice support, catheter care, and memory care Assist with meal preparation, feeding, laundry, light cleaning, and maintaining a safe and clean environment in accordance with home & community care standards Observe patient conditions closely for changes or concerns related to health or safety and communicate findings promptly to supervising healthcare providers Provide compassionate companionship while respecting patient privacy and promoting emotional well-being during visits Skills Strong knowledge of medical terminology and procedures relevant to home health caregiving Proven experience working with people with disabilities, developmental delays, or in assisted living environments Skilled in taking accurate vital signs and performing patient assessments with attention to detail Familiarity with EMR/EHR systems for medical records management and documentation accuracy Ability to operate medical equipment such as Hoyer lifts or other mobility aids safely and effectively Excellent communication skills for collaborating with clients, families, and healthcare teams Demonstrated caregiving expertise in long-term care settings including nursing homes or hospice environments This role is ideal for motivated individuals passionate about delivering exceptional patient care within a supportive team environment.
