Job Summary We are seeking a compassionate and dedicated Personal Care Assistant to provide exceptional support and care to individuals in various settings, including home health, assisted living, nursing homes, and memory care facilities. Responsibilities Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support using proper patient transfer methods and manual handling techniques like Hoyer lifts. Monitor patients' vital signs and observe for any changes in health status or behavior, documenting findings accurately in care plans. Support medication administration under supervision, ensuring compliance with HIPAA regulations and resident rights. Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption as needed. Maintain a clean and safe environment by performing light cleaning duties and ensuring infection control standards are upheld. Provide emotional support through active client communication, companionship, and engagement in meaningful activities. Assist with transportation for appointments or community outings using appropriate personal transportation assistance methods. Experience Prior experience working with individuals with disabilities, developmental or intellectual disabilities support, or senior care is highly desirable. Experience working in group homes, assisted living facilities, nursing homes, or home health environments is preferred. Knowledge of dementia care, Alzheimer's care, memory care, and neurodevelopmental disorders such as autism is a plus. Certification in First Aid and CPR is required; additional certifications like Hoyer lift operation or medication administration are advantageous. Familiarity with patient monitoring techniques, patient transfer methods, and the use of medical equipment such as Hoyer lifts or blood pressure cuffs is beneficial. Demonstrated ability to follow care plans accurately while respecting residents' rights and maintaining confidentiality under HIPAA guidelines.
