Duties Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts or transfer belts Monitor patient observations, including vital signs and behavioral changes, to ensure safety and well-being Administer medications and treatments following medical guidelines and documentation protocols in compliance with HIPAA regulations Support patients with specialized needs such as dementia, Alzheimer's disease, or other memory care requirements through tailored activities and compassionate engagement Document medical records accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems; maintain detailed care logs and reports Assist with catheter care, wound management, hospice support, and other complex medical procedures as trained Prepare nutritious meals and assist with feeding while adhering to dietary restrictions; perform light cleaning and laundry to maintain a safe environment Work collaboratively with healthcare teams to coordinate patient care plans and ensure seamless service delivery in home or community settings Skills Strong knowledge of medical terminology, medication administration, and patient care procedures Experience working with individuals with disabilities or developmental challenges in long-term care or home health environments Proficiency in using EMR/EHR systems for documentation and communication Excellent patient observation skills to detect changes in condition or behavior promptly Ability to assist with ADLs safely using mobility aids such as Hoyer lifts or transfer devices Familiarity with dementia, Alzheimer's, memory care, hospice care, and dementia-specific protocols CPR certification and knowledge of emergency response procedures for patient safety Compassionate caregiving attitude combined with strong communication skills for engaging patients effectively Reach out at : info@reliahomecare.com This role is ideal for dedicated professionals eager to provide high-quality care in dynamic settings.
