Job Description
Responsible for assessment of all psychosocial factors and participation in treatment planning of patients/families.
Required Knowledge & Skills
Education:
Bachelor's degree
Experience:
One year of related experience. Requires a working knowledge of standard practices and procedures.
Licenses and Certifications
- Driver's License
Qualifications - Bachelors Degree in Social Work required.
- Experience - minimum of one year in hospital or medical setting.
- Ability to deal comfortably with people in emotional need in a competent, calm manner.
- Proficiency in communication skills at all levels.
- Proficiency in observation, assessment and problem-solving skills.
- Demonstrates flexibility, initiative and organization in performance.
- Ability to work independently.
- CPR BLS certification or must be obtained within 30 days of hire
- Degree in social work and currently licensed or pursuing licensure as a social worker in the state of GA preferred.
- One year of home health experience preferred
