In this vital role, you will provide attentive, respectful, and personalized assistance to promote independence, dignity, and well-being. Whether working in a home setting, assisted living facility, or community environment, your dedication will help clients thrive and feel valued every day. Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and toileting to ensure comfort and hygiene. Support residents with mobility and transfers using manual handling techniques and equipment such as Hoyer lifts or patient transfer methods. Monitor patient health by taking vital signs, observing for changes in condition, and reporting findings accurately to healthcare professionals. Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption when necessary. Maintain detailed care plans, document patient progress, and update records in compliance with HIPAA regulations to ensure confidentiality. Administer medication as prescribed, following proper procedures for medication administration and documentation. Provide emotional support and companionship to foster a positive environment for residents or clients. Requirements Prior experience working in group homes, assisted living facilities, nursing homes, or home health care is highly desirable. Knowledge of patient observation techniques, dementia care (including Alzheimer's care), memory care, and hospice care practices. Familiarity with patient monitoring tools such as vital signs measurement and patient personal care assistance. Ability to perform manual handling tasks safely, including heavy lifting and the use of assistive devices like Hoyer lifts. Certification in CPR and First Aid is preferred; ongoing training in HIPAA compliance is essential. Experience working with individuals with developmental disabilities, neurodevelopmental disorders, autism spectrum disorder, or intellectual disabilities is advantageous. Strong communication skills to effectively interact with clients, families, and healthcare teams while maintaining professionalism and empathy.
