Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using proper patient transfer methods such as Hoyer lifts and manual handling techniques. Monitor patients' vital signs and observe for any changes in health status or behavior, documenting findings accurately while adhering to HIPAA regulations to ensure confidentiality. Prepare nutritious meals tailored to individual dietary needs and assist with meal planning and preparation to promote healthy eating habits. Support clients with medication administration following prescribed care plans and ensure timely delivery of prescribed treatments. Provide companionship and emotional support while engaging clients in meaningful activities that enhance mental well-being, including dementia or Alzheimer's care and memory care strategies. Maintain a clean and safe environment by performing light cleaning duties and ensuring proper infection control practices are followed at all times. Facilitate safe patient transfers using appropriate equipment such as Hoyer lifts or other transfer methods; assist with transportation for medical appointments or community outings when needed. Document all care activities thoroughly in client records, communicate effectively with healthcare teams, and participate in ongoing training related to hospice care, neurodevelopmental disorders, or developmental disabilities. Qualifications Proven experience working with individuals requiring personal care or support services; group home experience or assisted living experience is highly desirable. Knowledge of patient observation techniques, medication administration, vital signs measurement, and basic first aid/CPR certification. Familiarity with HIPAA regulations to maintain client confidentiality at all times. Experience working with diverse populations including seniors, individuals with dementia or Alzheimer's disease, autism spectrum disorder, intellectual disabilities, or neurodevelopmental disorders. Ability to perform heavy lifting safely and use manual handling equipment such as Hoyer lifts for patient transfers. Strong communication skills to effectively interact with clients, families, healthcare professionals, and team members. Demonstrated ability to follow detailed care plans while adapting to individual needs within home health or long-term care settings. Valid driver's license preferred for personal transportation assistance; flexibility to work various shifts including evenings or weekends as required.
