Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor patients' health status by taking vital signs, observing for changes, and reporting findings accurately Support medication administration following care plans and HIPAA confidentiality standards Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary Use proper patient transfer methods such as Hoyer lifts and manual handling techniques to ensure safety during mobility tasks Maintain a clean and safe environment through regular cleaning, laundry, and organization of personal spaces Document care activities, patient observations, and updates in compliance with healthcare regulations and facility policies Requirements Proven experience working with individuals with disabilities, developmental or intellectual disabilities support, or senior care preferred Knowledge of patient monitoring techniques including vital signs, dementia care, Alzheimer's care, memory care, and hospice care practices Ability to assist with complex needs such as assisted living or long-term care environments; experience with residents' specific needs is a plus Familiarity with patient transfer methods like Hoyer lifts and patient personal care assistance techniques Certification in First Aid and CPR required; additional certifications in medication administration or manual handling are advantageous Strong communication skills for effective client interaction and team collaboration Ability to lift heavy objects safely and perform manual handling tasks as needed Experience working with neurodevelopmental disorders or individuals with autism is highly valued Join us in providing exceptional home & community care that truly makes a difference!
