Duties Assist residents with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support using proper patient transfer methods like Hoyer lifts or manual handling techniques. Monitor patients' health by taking vital signs, observing patient conditions, and reporting changes promptly to healthcare professionals. Administer medications accurately following care plans and ensure compliance with HIPAA regulations to protect patient privacy. Support residents with meal preparation, feeding assistance, and maintaining proper nutrition aligned with individual dietary needs. Engage in patient observation and communication to foster trust, understanding, and emotional support for individuals with developmental disabilities, autism, dementia, or neurodevelopmental disorders. Maintain a clean and safe environment by performing light cleaning duties and adhering to infection control protocols. Provide transportation assistance for appointments or community outings while ensuring safety during transfers and outings. Qualifications Prior experience working in group homes, assisted living facilities, nursing homes, or home health care is highly preferred. Knowledge of patient care techniques including medication administration, vital signs monitoring, first aid, CPR certification, and use of Hoyer lifts or other patient transfer methods. Experience supporting individuals with Alzheimer's disease, dementia care, memory care needs or those with intellectual or developmental disabilities. Familiarity with HIPAA regulations and maintaining confidentiality of patient information. Ability to perform heavy lifting safely and handle manual tasks required for patient transfers or mobility support. Strong communication skills to effectively interact with clients, families, and healthcare teams. Valid certifications in first aid and CPR are required; additional certifications in hospice care or home & community care are a plus.
