Responsibilities Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility assistance Monitor patient health by observing vital signs such as blood pressure, pulse, respiration, and temperature Administer medications accurately following medical instructions and document all medical records in Electronic Health Record (EHR) systems Support patients with specialized care needs such as Alzheimer's disease, dementia, hospice care, and end-of-life care Utilize patient transfer methods safely using equipment like Hoyer lifts and proper patient transfer techniques Maintain patient confidentiality in compliance with HIPAA regulations while documenting medical records and progress notes Perform light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe environment for clients Assist with medical procedures such as catheter care, wound care, and assisting with medical documentation when necessary Communicate effectively with clients and family members to ensure personalized care plans are followed and needs are met Follow home healthcare regulations diligently to ensure safety standards are upheld at all times Experience Prior experience working with individuals with developmental disabilities or in assisted living settings is highly preferred Demonstrated experience in geriatrics or working with senior populations in home health or nursing home environments Knowledge of medical terminology, patient care techniques, and basic life support (BLS) including CPR certification Experience operating EMR (Electronic Medical Records) systems or EHR systems for accurate documentation Familiarity with patient transfer methods such as Hoyer lifts or other mobility aids is advantageous Proven ability to handle heavy lifting safely while assisting patients during transfers or mobility activities Previous roles involving hospice or long-term care provide valuable insight into end-of-life support needs Join us in delivering compassionate home health care that truly transforms lives!
