Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support. Monitor and observe patients' conditions, accurately documenting vital signs and medical information using Electronic Health Record (EHR) systems while maintaining compliance with HIPAA regulations. Support medication administration and manage catheter care, ensuring safety and adherence to medical protocols. Provide specialized care for clients with dementia, Alzheimer's disease, or other memory-related conditions, including memory care techniques. Operate medical equipment such as Hoyer lifts and assist with transfers to ensure patient safety during mobility assistance. Prepare nutritious meals tailored to dietary restrictions and assist with light housekeeping tasks such as cleaning, laundry, and meal preparation to maintain a safe living environment. Collaborate with healthcare professionals and family members to develop comprehensive care plans that meet individual needs. Maintain professionalism by adhering to all safety standards, including CPR certification and proper medical documentation practices for hospice or home health care. Experience Prior experience in home health care, nursing homes, assisted living facilities, or long-term care environments is highly desirable. Proven ability to work effectively with individuals who have developmental disabilities or cognitive impairments such as dementia or Alzheimer's disease. Familiarity with EMR (Electronic Medical Records) or EHR systems for accurate documentation of patient information. Knowledge of medical terminology relevant to patient care and medication management. Experience in providing hospice or palliative care is advantageous. Demonstrated competency in vital signs measurement, medication administration, and catheter care procedures. Strong caregiving skills complemented by excellent communication abilities and a compassionate approach to patient support.
