Responsibilities Deliver compassionate assistance with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support using tools like Hoyer lifts and other assistive devices Monitor and record vital signs including blood pressure, pulse, temperature, and respiration to track health status accurately Assist with medication administration following proper protocols and maintaining detailed medical documentation in Electronic Health Record (EHR) systems Observe patients closely for changes in condition, behavior, or symptoms, especially in dementia or Alzheimer's care settings, ensuring timely reporting to healthcare professionals Support patients with specialized needs such as catheter care, hospice care, or working with individuals with developmental disabilities or disabilities Maintain a clean and safe environment by assisting with light housekeeping tasks like laundry and meal preparation in accordance with health standards Document all patient interactions thoroughly while respecting HIPAA regulations to ensure confidentiality and compliance Requirements Valid certification as a CNA or equivalent credential; experience as a Personal Care Assistant is highly valued Proven experience working in assisted living facilities, nursing homes, or home health environments; long-term care experience preferred Familiarity with EMR/EHR systems for accurate medical documentation and record keeping Knowledge of medical terminology relevant to patient care and medication administration procedures Ability to perform patient observation diligently and respond promptly to changing needs or emergencies CPR certification is required; additional certifications such as hospice care, dementia care, or Alzheimer's care are advantageous Strong communication skills to collaborate effectively with healthcare teams and connect empathetically with patients Join us in creating a nurturing environment where every individual receives the attentive care they deserve.
