Duties Assist residents with activities of daily living (ADLs), including bathing, dressing, grooming, and personal hygiene to promote comfort and independence. Monitor patients' health by taking vital signs, observing for changes, and reporting findings to healthcare professionals to ensure optimal care. Support medication administration in accordance with care plans and HIPAA regulations to maintain safety and compliance. Prepare nutritious meals and assist with meal planning to meet dietary needs of residents with various health conditions. Engage residents in meaningful activities, socialization, and community outings to enhance quality of life. Maintain a clean, safe environment by performing light housekeeping tasks including cleaning common areas and resident rooms. Document care activities accurately in resident records while adhering to confidentiality standards like HIPAA. Support residents with specialized needs such as dementia, Alzheimer's, autism, or developmental disabilities through tailored approaches and patience. Provide emotional support and companionship to foster trust and promote mental well-being during interactions. Skills Proven experience working with individuals with disabilities or neurodevelopmental disorders such as autism or intellectual disabilities. 2 years minimum. Strong knowledge of patient monitoring techniques including vital signs, patient observation, and dementia or memory care practices. Familiarity with assisted living environments, nursing homes, home health care, hospice care or long-term care settings. Ability to perform patient personal care assistance including assisting with ADLs and using transfer methods like Hoyer lifts safely. Knowledge of medication administration procedures along with CPR and first aid certifications for emergency response readiness. Excellent communication skills for engaging effectively with clients, families, healthcare teams, and community resources. Ability to lift heavy objects safely during manual handling tasks while maintaining proper body mechanics. Experience in meal preparation, personal transportation assistance, and home & community care services is highly valued. Comprehension of HIPAA regulations ensuring confidentiality of all patient information at all times.
