Job Summary We are seeking a compassionate and dedicated Home Care Aide to provide exceptional support and care to individuals in their homes or community settings. Responsibilities Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and personal hygiene. Support patients with mobility and transfers using appropriate techniques and equipment such as Hoyer lifts or patient transfer methods. Monitor vital signs, observe patient conditions, and report any changes promptly to healthcare professionals. Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption when necessary. Medication reminders Provide companionship and emotional support, fostering positive communication and respecting residents' rights. Maintain a clean and safe environment by performing light housekeeping tasks such as cleaning, laundry, and organizing personal spaces. Support clients during outings or appointments by providing personal transportation assistance and ensuring safety during travel. Follow all safety protocols including first aid, CPR, manual handling, and emergency procedures to ensure client safety at all times. Experience Prior experience working in home care , or as CNA ,assisted living facilities, nursing homes, or group homes is highly preferred. Demonstrated experience in patient observation, dementia care (including Alzheimer's care), memory care, or hospice care is advantageous. Must have recent TB and CPR Familiarity with care plans, patient monitoring, and resident's rights is essential Knowledge of HIPAA regulations and home & community care best practices is required. Proven ability to handle heavy lifting safely using proper manual handling techniques is important. Strong communication skills for client interaction and team collaboration are necessary for success in this role.
