Job Description
Overview Join our dedicated healthcare team as a Home Health Aide, where your compassionate care and attention to detail will make a meaningful difference in the lives of our clients. In this vital role, you will provide personalized assistance to individuals in their homes, supporting their health, safety, and well-being. Your energy, empathy, and professionalism will help promote independence and improve quality of life for those you serve. This position offers a rewarding opportunity to work closely with diverse patient populations, including seniors, individuals with disabilities, and those receiving hospice or memory care services. Duties Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support Monitor patient health by observing vital signs such as blood pressure, pulse, respiration, and temperature; report any concerns promptly Support medication administration following prescribed schedules and protocols while maintaining accurate medical documentation Provide specialized care for patients with dementia or Alzheimer's disease, ensuring safety and comfort through memory care techniques Operate Electronic Health Record (EHR) systems to document patient interactions, medical records, and care plans accurately Assist with transfers using patient transfer methods such as Hoyer lifts or other assistive devices to ensure safe mobility Maintain patient confidentiality in compliance with HIPAA regulations while providing home health care assistance and personal care support Requirements Prior experience working in assisted living facilities, nursing homes, or home healthcare settings is highly preferred Demonstrated ability in patient observation and recognizing changes in patient condition Knowledge of medical terminology, catheter care, wound management, and end-of-life care practices Certification in CPR (Cardiopulmonary Resuscitation) and Basic Life Support (BLS) required; first aid certification is a plus Familiarity with home health regulations and compliance standards necessary for delivering safe home & community care services Ability to lift heavy objects safely and perform physical tasks such as cleaning, laundry, meal preparation, and light housekeeping duties Experience working with individuals with developmental disabilities or geriatrics is advantageous Strong communication skills for effective client interaction and documentation; proficiency with EMR (Electronic Medical Records) systems preferred Valid driver's license and reliable transportation may be required for client visits Join us to provide compassionate home health services that truly improve lives!
