Job Description
Paladin seeks a Direct Support Professional for our Lake County Day Service program. In this on-site role, you will support individuals with diverse needs by fostering independence, life skills, and community engagement. You'll implement service plans, assist with daily living activities, run group and individual activities, and maintain accurate documentation. Working within Paladin's collaborative, inclusive culture, you'll ensure client safety, dignity, and growth while benefiting from training, career development, and a supportive team environment. Responsibilities Provide direct support and supervision to clients in a structured day service setting Assist clients with daily living skills, social engagement, and community integration Implement individualized service and support plans in collaboration with care teams Monitor, document, and report client progress, behaviors, and incidents accurately Ensure a safe, respectful, and inclusive environment aligned with Paladin's values Coordinate activities that promote independence, skill development, and wellness Communicate effectively with families, caregivers, and interdisciplinary team members Follow regulatory, health, and safety guidelines and organizational policies Required Skills Client care and supervision Behavioral support and de-escalation Documentation and recordkeeping Person-centered planning Basic healthcare and safety practices Crisis prevention awareness Time management and organization Interpersonal and communication skills Conflict resolution Basic computer and reporting skills
