Duties Provide assistance with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor and observe patients' health status, recording vital signs such as blood pressure, pulse, and temperature accurately using Electronic Health Record (EHR) systems Assist with medication administration following prescribed instructions while maintaining strict adherence to home health care regulations and HIPAA confidentiality standards Support patients with specialized needs such as Alzheimer's care, dementia care, memory care, and end-of-life care Use patient transfer methods and equipment like Hoyer lifts safely to ensure patient mobility and prevent injury during transfers Maintain a clean and safe environment by assisting with light housekeeping tasks such as laundry, meal preparation, and general cleaning Document all patient interactions thoroughly in medical records and communicate effectively with healthcare teams regarding patient progress or concerns Qualifications Prior experience working with individuals with disabilities, developmental disabilities, or geriatric patients is highly preferred Knowledge of medical terminology, catheter care, and vital signs measurement is essential Certification in First Aid and Basic Life Support (BLS), including CPR training Demonstrated experience with EMR (Electronic Medical Records) or EHR systems for accurate documentation Ability to perform heavy lifting when necessary and work efficiently within home health agency protocols Strong communication skills for client interaction and team collaboration Valid driver's license and reliable transportation are often required for community-based visits Compassionate caregiving attitude with a commitment to maintaining patient confidentiality under HIPAA guidelines Embark on a fulfilling career that empowers you to provide essential care while developing your skills in a supportive environment.
