Job Description

Job Summary We are seeking a compassionate and dedicated Personal Care Assistant to provide exceptional support and care to individuals in various settings, including assisted living, home health, and community care environments. This vital role involves assisting clients with daily activities, ensuring their safety, and promoting their independence. You will work closely with clients to deliver personalized care that enhances their quality of life while adhering to all safety and privacy standards. Responsibilities Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support using equipment like Hoyer lifts when necessary Monitor patients' vital signs and observe for any changes in health status or behavior Prepare nutritious meals tailored to individual dietary needs and assist with feeding when required Administer medications accurately following prescribed care plans while maintaining compliance with HIPAA regulations Support clients during outings or community activities to promote social engagement and independence Maintain detailed documentation of care provided, patient observations, and any incidents or concerns Perform light cleaning duties to ensure a safe and hygienic environment for clients Qualifications Prior experience working in group homes, nursing homes, assisted living facilities, or home & community care settings is highly preferred Knowledge of patient observation techniques, medication administration, and vital signs monitoring Certification in First Aid and CPR is required; additional certifications such as HIPAA training or dementia/Alzheimer's care are advantageous Ability to lift heavy objects safely and assist with mobility using proper techniques or equipment like Hoyer lifts Experience working with individuals with developmental disabilities, autism, dementia, or memory care needs is a plus Strong communication skills with a compassionate approach to patient care Must be reliable, detail-oriented, and committed to maintaining confidentiality and adhering to all safety protocols Join our team to provide meaningful support that empowers individuals to live healthier, happier lives.
