This role involves assisting individuals with daily activities, ensuring their safety, and supporting their overall well-being within a home or community setting. The ideal candidate will have experience in patient care, familiarity with medical terminology, and a strong commitment to improving quality of life for those they serve. This position offers the opportunity to work closely with clients, including seniors, individuals with disabilities, and those requiring specialized care such as Alzheimer's or dementia care. Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor and observe patient conditions, reporting changes or concerns promptlys Utilize Electronic Health Record (EHR) systems to document care activities accurately Support patients with memory care needs, Alzheimer's, dementia, and hospice care as required Operate medical equipment such as Hoyer lifts and perform catheter care safely Prepare meals and assist with meal planning tailored to dietary needs Perform light housekeeping tasks including laundry and cleaning to maintain a safe environment Assist with transportation to appointments or community activities when necessary Follow all safety procedures and infection control protocols Qualifications Prior experience in home health, assisted living, nursing homes, or long-term care settings preferred Knowledge of medical terminology and medical documentation practices Certification in CPR is required; additional certifications such as hospice or dementia care are a plus Experience working with individuals with developmental disabilities or disabilities is advantageous Ability to work independently while maintaining high standards of patient care Strong communication skills and compassion for patients of all ages and backgrounds Familiarity with EMR/EHR systems and HIPAA compliance standards Ability to lift and assist patients using equipment such as Hoyer lifts safely Valid driver's license preferred for community-based services This role is vital in supporting the health and independence of clients within their homes.
