Job Overview We are seeking a dedicated and compassionate Home Health Aide & Caregiver to provide exceptional in-home care to individuals with diverse needs. The ideal candidate will possess a strong commitment to patient well-being, with experience in supporting seniors, individuals with disabilities, and those requiring specialized medical care. This role offers an opportunity to make a meaningful difference in clients' lives by delivering personalized, respectful, and professional care within their home or community settings. Location Pepperall MA and surrounding towns Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support. Monitor and observe patients' health status, accurately documenting vital signs and changes in condition using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems. Administer medications safely following medical instructions and document medication administration in compliance with HIPAA regulations. Support clients with specialized care needs such as Alzheimer's, dementia, memory care, hospice care, and catheter care. Operate assistive devices including Hoyer lifts and other mobility aids to ensure safe transfers and prevent injury. Prepare nutritious meals tailored to dietary restrictions and assist with meal planning and cooking. Perform light housekeeping tasks such as cleaning, laundry, and maintaining a safe environment to promote overall well-being. Collaborate effectively with healthcare teams to ensure continuity of patient care and adherence to medical terminology standards. Provide compassionate support for clients with developmental disabilities or long-term care needs within assisted living or home settings. Maintain patient confidentiality and adhere strictly to HIPAA guidelines while managing medical records and documentation. Experience Prior experience working in home health, assisted living facilities, nursing homes, or long-term care environments is highly desirable. Proven ability to work with individuals with disabilities, Alzheimer's disease, dementia, or memory impairments. Familiarity with EMR/EHR systems for accurate documentation of patient information and medical records. Knowledge of medical terminology relevant to caregiving tasks and patient health monitoring. Certification in CPR is required; additional training in hospice care or advanced caregiving skills is advantageous. Experience operating medical equipment such as Hoyer lifts or vital signs monitors is preferred. Demonstrated ability to provide patient-centered care while maintaining professionalism, patience, and empathy.
