This paid position offers a rewarding opportunity to work closely with diverse patient populations, including seniors, individuals with disabilities, and those receiving hospice or palliative care. Responsibilities Deliver comprehensive personal care assistance, including bathing, dressing, grooming, and mobility support Assist with Activities of Daily Living (ADLs) such as toileting, feeding, and medication reminders Monitor patient health by observing vital signs and reporting changes promptly using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems Support patients with mobility using patient transfer methods and equipment like Hoyer lifts or gait belts while ensuring safety protocols are followed Administer medications accurately in accordance with medical documentation and home healthcare regulations Provide specialized care for patients with dementia or Alzheimer's disease, including memory care techniques and behavioral management Maintain patient confidentiality by adhering to HIPAA guidelines and safeguarding medical records and sensitive information Assist with light housekeeping tasks such as cleaning, laundry, and meal preparation to promote a safe and comfortable environment Support end-of-life care with compassion, dignity, and respect for patients' wishes Communicate effectively with clients and their families to ensure understanding of care plans and address concerns Qualifications Proven experience working as a Home Health Aide or in assisted living/nursing home settings preferred Strong knowledge of medical terminology, medication administration, catheter care, and vital signs monitoring Experience working with individuals with developmental disabilities or dementia is highly desirable Familiarity with EMR/EHR systems for documentation and client communication Certification in First Aid and Basic Life Support (BLS), including CPR training required; additional certifications in hospice or geriatrics are a plus Ability to lift heavy objects safely and perform physically demanding tasks such as patient transfers using Hoyer lifts or other equipment Knowledge of home health care regulations and compliance standards such as HIPAA is essential Excellent communication skills to foster trustful relationships with clients and their families Valid driver's license and reliable transportation preferred for client visits Join us in providing exceptional home health care that empowers individuals to live healthier, happier lives. $16.00 - $16.50 per hour
