This position offers an opportunity to make a meaningful difference in the lives of patients by assisting with daily activities, ensuring safety, and promoting independence. You will work closely with clients, families, and healthcare professionals to deliver comprehensive home health services that foster comfort, dignity, and well-being. Duties Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support using equipment like Hoyer lifts when necessary Prepare nutritious meals and assist with feeding as needed; perform light housekeeping tasks including cleaning and laundry to maintain a safe environment Document all patient interactions accurately in medical records, ensuring compliance with healthcare standards and confidentiality protocols Experience Prior experience working in assisted living facilities, nursing homes, or home health environments is highly preferred Familiarity with EMR (Electronic Medical Records) or EHR (Electronic Health Records) systems for medical documentation Knowledge of medical terminology and procedures related to patient care and safety protocols including CPR certification Experience providing long-term care or hospice services is advantageous Ability to work independently in a home setting while maintaining high standards of patient care and safety Strong communication skills to collaborate effectively with healthcare teams, patients, and families Join us in delivering compassionate home health services that empower individuals to live comfortably and confidently in their own homes.
