Job Summary We are seeking a compassionate and dedicated Caregiver/Personal Care Assistant to join our team in providing exceptional support to individuals with diverse needs. In this vital role, you will assist clients with daily activities, ensure their safety, and promote their independence and well-being. Duties Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and personal hygiene. Monitor patients' health status by observing vital signs and overall condition, reporting any changes promptly. Support medication administration following care plans and HIPAA regulations to ensure privacy and safety. Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption if necessary. Facilitate patient transfers using proper manual handling techniques and equipment such as Hoyer lifts or patient transfer methods. Maintain a clean and safe environment through regular cleaning, sanitation, and organization of client spaces. Provide transportation assistance for appointments, errands, or community outings to promote independence and engagement. Requirements Spanish Speaking Knowledge of patient observation techniques, dementia care (including Alzheimer's care), memory care, and hospice care practices. Ability to perform heavy lifting safely and assist with manual handling tasks using proper patient transfer methods Strong communication skills for effective client interaction and documentation of care plans or progress notes.
