Responsibilities Provide personal care assistance including bathing, dressing, grooming, and mobility support to clients Assist with Activities of Daily Living (ADLs) such as toileting, feeding, and medication reminders Monitor and record vital signs accurately using medical terminology and EMR (Electronic Medical Records) systems Support patients with memory care needs, dementia, Alzheimer's care, and hospice or end-of-life care as required Ensure patient safety through proper transfer methods using equipment like Hoyer lifts or patient transfer techniques Maintain strict confidentiality of medical records in compliance with HIPAA regulations Perform light housekeeping tasks including cleaning, laundry, meal preparation, and maintaining a safe environment Skills Fluent in Punjabi with excellent client communication skills Proven experience in assisted living, geriatrics, or home health care environments Knowledge of medical terminology, medication administration, catheter care, and vital signs monitoring Familiarity with home health regulations and documentation procedures within EMR/EHR systems Ability to perform patient transfers safely using appropriate methods and equipment such as Hoyer lifts Strong caregiving skills with experience working with individuals with disabilities or developmental challenges Certified in First Aid and CPR; basic life support certification preferred Capable of heavy lifting and physical activity required for patient transfer and mobility assistance Experience working in hospice or end-of-life care settings is highly desirable Demonstrated ability to maintain patient confidentiality while providing compassionate care Join our team today to provide vital support that enhances the quality of life for our clients.
