Job Description
Job Summary Join our compassionate healthcare team as a Home Health Aide, where you'll play a vital role in providing personalized, high-quality care to individuals in their homes. This energetic position offers the opportunity to support patients with a variety of needs, including assistance with daily activities, medication management, and emotional support. Duties Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility Monitor and observe patient health status, including vital signs and overall condition Administer medications following prescribed instructions and document medical records accurately using Electronic Health Record (EHR) systems Support patients with specialized care needs including Alzheimer's care, dementia care, hospice support, and working with individuals with developmental disabilities or disabilities Operate medical equipment such as Hoyer lifts, catheters, and other assistive devices safely and effectively Prepare nutritious meals and assist with feeding while adhering to dietary restrictions and medical guidelines Perform light housekeeping tasks including cleaning, laundry, and maintaining a safe environment for the patient Document all patient interactions thoroughly while respecting HIPAA regulations to ensure confidentiality and privacy Collaborate with healthcare teams to coordinate care plans and update medical documentation regularly Provide emotional support and companionship to promote mental well-being Experience Prior experience in home health care or assisted living environments is highly preferred Demonstrated ability to work with seniors or individuals with disabilities in long-term care settings Experience using EMR (Electronic Medical Records) or EHR (Electronic Health Records) systems for documentation Knowledge of medical terminology relevant to patient care and medication administration Certification in CPR (Cardiopulmonary Resuscitation) is required; additional certifications such as hospice care or dementia training are advantageous Familiarity with medical procedures including vital signs measurement, catheter care, and hospice support techniques Previous work experience in nursing homes or assisted living facilities is beneficial but not mandatory—training will be provided for dedicated candidates eager to learn Join us as a Home Health Aide and become an essential part of a caring community dedicated to enriching lives through compassionate home-based healthcare.
