A Home Health Aide helps individuals who are disabled, elderly, or chronically ill with daily tasks, such as dressing, washing, and light housekeeping. They can care for patients in their homes or other healthcare settings, and may perform basic medical care under the direction of a nurse or other medical professional. Job Summary We are seeking a compassionate and dedicated Part-Time Care Services Assistant to join our team, providing essential support to our case managers/nursing staff. This position will work M-F 9am-1pm answering phones, relaying messages, reaching out to clients, data entry, and other office duties as assigned.
Nature and Scope of Job:
Position requires a variety of data entry tasks related to clients, staff, volunteers, and programs. Daily functions will utilize word processing, spreadsheets and database functions. Position requires detail work, great organizational abilities and excellent communication skills.
Essential Job Functions:
Home Care 1.
Maintains client file system:
a. Creates file for new clients and sends out forms as appropriate. b. Updates files, filing inactive or deceased clients in the appropriate filing system. 2. Enters and updates information in the agency's computer system. a. Enters and updates information on all clients to include assessments, reassessments, demographics and other information, as directed. 3. Assists with scheduling Home Care Aides in clients' homes. a. Assists with maintaining scheduling calendars for home care aides, making sure all appointments are confirmed. b. Tracks cancellations of services with dates and reasons and faxes the 'Non-services' forms to third party organizations. 4. Supplies and maintains forms for Home Care Department. a. Prepares Home Care assessment packets for case managers. b. Orders or copies all forms needed for the Home Care Department. 5. Performs routine typing and photocopying as assigned for the Home Care department 6. Assists with special projects and coordinates delegated aspects of special projects. 7. Assist with support groups; scheduling guest speakers, researching support group topics, lead support groups as needed. 8. Assists with Project Fresh, as needed. 9. Attends relevant community meetings, seminars and trainings upon the discretion of the Care Service Team Leader. 10. Follows the agency's safety and health procedures to ensure safe working practices on the job and in the client's homes. 11. Performs other duties as assigned.
Minimum Qualification:
Education:
High school diploma, GED or equivalent work experience is required with courses in word processing, spreadsheets and data entry functions. Associate degree or college courses preferred.
Experience:
Two years plus of progressively more responsible experience in human services is preferred. Experience in home health care services or gerontology preferred.
Other:
May require a car and current driver's license.
Certifications:
Must maintain current CPR, First Aid and AED certification (provided)
