Duties Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support Monitor patient health by observing vital signs such as blood pressure, pulse, respiration, and temperature; report changes to healthcare professionals promptly Administer medications accurately following prescribed instructions and document medication administration in medical records or electronic health record (EHR) systems Support patients with specialized needs such as Alzheimer's care, dementia care, end-of-life care, and memory care with patience and empathy Perform catheter care and other medical procedures as trained while maintaining strict adherence to HIPAA regulations to ensure patient confidentiality Assist with patient transfer methods using equipment like Hoyer lifts or other safe techniques to prevent injury Maintain a clean and safe environment by assisting with light housekeeping tasks such as laundry, cleaning, and meal preparation in accordance with home health care standards Document all patient interactions thoroughly using medical documentation protocols; ensure compliance with home health agency policies and home healthcare regulations Provide emotional support and effective communication to patients and their families to foster trust and understanding Skills Strong patient observation skills to identify changes in condition or behavior promptly Prior experience assisting living or long-term care environments; familiarity with geriatric populations preferred Proficiency with EMR (Electronic Medical Records) or EHR systems for accurate documentation Knowledge of medical terminology relevant to home healthcare settings Experience working with individuals with developmental disabilities or disabilities in general; dementia or Alzheimer's experience highly valued Ability to perform basic life support (BLS), CPR, first aid, and catheter care confidently Familiarity with home health care assistance techniques including patient transfer methods and medication management Understanding of home health regulations, HIPAA compliance, and patient confidentiality standards Physical ability for heavy lifting when necessary; capable of assisting with mobility tasks safely Excellent client communication skills; compassionate caregiving approach that respects patient dignity Embark on a fulfilling career where your skills directly impact the well-being of those in need.
