Full Time | 40 hours per week | 11 am
  • 7:30 pm | Monday
  • Friday, with alternating weekends

LifeCircles PACE is looking for a dedicated and compassionate Home Health Aide (HHA) to join our PACE (Program of All-Inclusive Care for the Elderly) team. In this role, you will provide vital in-home care and support that empowers older adults to remain safely in their homes while maintaining their dignity and independence.   • $20.43 / hour Reporting To Samantha Jaggassar Key Responsibilities Provide Hands-On Care in Their Home
  • Assist with personal care (ADLS) like bathing, grooming, dressing, and toileting while following the participant's Life Plan. Maintain Comfort & Care
  • Support housekeeping tasks such as laundry, sweeping, and grocery shopping Support Mealtime
  • Help participants enjoy their meals and stay hydrated for good health. Keep Things Moving
  • Support mobility, including turning, positioning, and safe transfers. Perform Vital Checks
  • Measure and record vital signs, weight, and intake/output. Maintain Comfort & Care
  • Keep rooms tidy, cozy, and well-stocked with essentials. Provide Responsive Support
  • Respond with kindness and compassion. Monitor with Watchful Eyes
  • Report any changes in conditions to the interdisciplinary team (IDT) and leadership ASAP. Demonstrate Respect & Professionalism
  • Maintain confidentiality while delivering top-notch care.
