Home Care Participant Care Assistant, CNA Schedule:
Full-time 40 hours per week | 8am
  • 4:30pm | Monday

Home Care | PACE of Southwest Michigan |
Van Buren County Travel Requirements:
This role involves regular travel providing Home-Based Care. You must live in Van Buren County for this position to be a feasible commute.
What You'll Do:
The Home Care Participant Care Assistant (PCA) provides essential in-home care and assistance to participants enrolled in the PACE program. You will work closely with the Home Care Supervisor to deliver compassionate, person-centered care and support activities of daily living. From assisting with daily activities to providing comfort and companionship, your work will make a meaningful impact on the lives of those we serve.   • $21.39 / hour Reporting To Teresa Ford Key Responsibilities Provide Hands-On Care in Their Home
  • Assist with personal care (ADLS) like bathing, grooming, dressing, and toileting. Support Mealtime
  • Help participants enjoy their meals and stay hydrated for good health. Keep Things Moving
  • Support mobility, including turning, positioning, and safe transfers. Perform Vital Checks
  • Measure and record vital signs, weight, and intake/output. Maintain Comfort & Care
  • Support housekeeping tasks such as laundry, sweeping, and grocery shopping. Provide Responsive Support
  • Answer call lights quickly and respond with kindness and compassion. Monitor with Watchful Eyes
  • Report any changes in conditions to the leadership ASAP. Demonstrate Respect & Professionalism
  • Maintain confidentiality while delivering top-notch care.

Skills, Knowledge and Expertise:
One (1) year of prior experience with frail or elderly individuals, aiding with daily living activities. Valid Michigan Driver's License with a clean driving record and reliable transportation. CNA certification, desired. Completion of the Participant Care Assistant (PCA) program at PACE of Southwest Michigan or an equivalent training program if not a Certified Nurse Assistant. Current CPR or BLS certification or willing to obtain. Ability to work independently, multitask, and operate as part of an interdisciplinary team. Benefits Career Growth & Development
