Job Overview 88 y.o female client needs daily assistance in her home. She is wheelchair bound and dependent for most ADLS. In this vital role, you will provide personalized support, ensuring safety, comfort, and dignity for clients in their homes or care facilities. Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support Monitor patient health through observation of vital signs and overall condition, reporting changes promptly Administer medications and perform catheter care following medical instructions and home healthcare regulations Support clients with meal preparation, light housekeeping, laundry, and maintaining a clean living environment Utilize Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to document care activities accurately and securely Employ patient transfer methods such as Hoyer lifts or other assistive devices to ensure safe mobility Provide compassionate end-of-life care or hospice support when needed, respecting patient wishes and confidentiality Qualifications Proven experience working with seniors, individuals with disabilities, or in assisted living environments Knowledge of medical terminology, medication administration, and basic life support (BLS) including CPR certification Familiarity with home health care assistance, dementia care, Alzheimer's care, memory care, and geriatric patient needs Ability to perform patient observation, vital signs measurement, and maintain patient confidentiality in accordance with HIPAA regulations Skilled in patient transfer methods and using assistive devices such as Hoyer lifts or transfer belts Strong communication skills for effective client interaction and team collaboration Valid certification in first aid and CPR; experience working within home health agencies preferred Ability to lift heavy objects safely and perform physical tasks required for caregiving duties Join us to provide essential home health services that promote independence, safety, and well-being.
