Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using proper manual handling and patient transfer methods such as Hoyer lifts. Monitor patients' vital signs and observe for any changes in health status, documenting findings accurately while respecting residents' rights and confidentiality under HIPAA guidelines. Prepare nutritious meals tailored to individual dietary needs, ensuring safe food handling and timely meal delivery. Support residents with medication administration following care plans and ensuring compliance with prescribed schedules. Maintain a clean and safe environment by performing light housekeeping duties, including cleaning personal spaces and common areas. Provide transportation assistance for appointments or community outings, ensuring safety during transfers and travel. Engage in patient monitoring and communication to report any concerns or changes to healthcare professionals promptly, especially in cases involving dementia, Alzheimer's, or neurodevelopmental disorders. Experience Prior experience working in group homes, assisted living facilities, nursing homes, or home health settings is highly preferred. Demonstrated ability to perform patient observation, monitor vital signs, and assist with medication administration accurately. Familiarity with care for individuals with dementia, Alzheimer's disease, autism spectrum disorder, intellectual disabilities, or developmental disabilities supports success in this role. Knowledge of HIPAA regulations and resident's rights ensures respectful and confidential care delivery. Experience with manual handling equipment such as Hoyer lifts and patient transfer techniques is advantageous. Proven skills in caregiving tasks including assisting with ADLs, meal preparation, personal transportation assistance, and light housekeeping.
