Job Description

Minn Care Home Health is looking for Direct Support Professionals for our Assisted Living in
New Hope, MN:
Thursday through Saturday (11pm - 7am) Starting at $18/hr Minn Care Home Health provides home care services to adults with developmental and mental disabilities, and those in need of medical care after hospital discharge. The Direct Support Professional will perform direct client cares ordered by the client's physician and as documented in the client's care plan.
MAIN RESPONSIBILITIES
A.
Assist clients with personal care:
dressing, grooming, bathing, eating, toileting, transferring, positioning, mobility. B.
Housekeeping:
cleaning all living areas, making the bed, dishes, laundry, taking out the garbage C. Monitor and re-direct client behaviors to ensure their safety. D. Meal planning and preparation. E.
Medications:
ordering, administration and monitoring. F. Arrange client transportation. G. Assist clients in getting around and participating in the community. H. Assist clients in managing their finances.
REQUIREMENTS A.
Must pass the Department of Human Services background check B. Must be reliable and have a strong work ethic D. Fluent in English and have good communication skills E. Must show compassion and empathy to clients F. Must have a driver's license
