Summary Join Homeward Bound as a Direct Support Professional in our community-focused team in Plymouth, MN . In this vital role, you will help individuals with disabilities achieve their personal goals by providing compassionate support and assistance. Your work will directly contribute to empowering our clients to live more independent and connected lives. Schedules Available Full time work is 30+hours/week: 4 weekdays + every other Sat/Sun Part time work is 12-29 hours/week: Weekends Only or 1-2 weekdays + every other
Sat/Sun Shifts Available:
Evening Shifts (3 PM
- 9 PM) Responsibilities Provide direct support to individuals with disabilities, assisting with daily activities and personal care.
Monitor health and safety by observing clients and responding to their needs. Assist with medication administration and maintaining care plans. Support clients in engaging with their community and participating in activities. Maintain a clean and safe environment within residential or community settings. Collaborate with team members to develop and implement individualized support plans. Document client progress and report any concerns to supervisors. Uphold confidentiality and adhere to HIPAA regulations at all times. Qualifications Experience working in group homes or similar settings supporting individuals with disabilities preferred, but not required. Must be able to communicate effectively in English verbally and in writing. Must be able to accurately count money. Must pass a MN Dept of Human Services Background Study. Compassionate attitude with excellent communication skills. Ability to assist with activities of daily living (ADLs) such as bathing, toileting, dressing, and meal preparation for both female and male residents. Familiarity with health monitoring techniques including vital signs. Ability to lift and transfer up to 50 lbs. Commitment to empowering clients and respecting their dignity.
