Part-time About the Role The Individualized Home Supports (IHS) Worker provides person-centered support to individuals in their home and community to help them maintain and improve their independence, health, safety, and quality of life. The IHS Worker provides services according to the individual's authorized service plan, individual service plan, support plan, and the policies and procedures of the 245D licensed provider. The worker supports the individual in developing and maintaining skills, participating in the community, making informed choices, and achieving identified goals. The IHS Worker must respect the individual's rights, dignity, privacy, preferences, culture, and choices at all times. Essential Duties and Responsibilities The IHS Worker will: Provide individualized, person-centered support based on the individual's assessed needs and authorized services. Follow the individual's service plan and provider policies and procedures. Assist the individual in developing, maintaining, and improving independent living skills. Support the individual with activities of daily living and household tasks when authorized in the individual's service plan. Assist with meal planning, meal preparation, grocery shopping, and other household activities as identified in the individual's plan. Support personal hygiene, grooming, dressing, and other personal care activities when authorized and within the worker's training and assigned responsibilities. Teach and reinforce skills related to: Household management Cooking and meal preparation Cleaning and organization Laundry Money management Shopping Use of public transportation Communication Social skills Community participation Health and wellness Personal safety Encourage the individual to make choices and exercise control over their daily routines and activities. Support individuals in accessing and participating in community activities. Assist the individual in developing and maintaining relationships with family, friends, neighbors, and other community members. Provide transportation or transportation assistance when authorized and permitted by the individual's plan and agency policy. Implement approved positive support strategies and follow applicable behavior support plans. Monitor and report changes in the individual's physical, emotional, behavioral, or mental well-being. Follow individual-specific health and safety procedures. Assist with medications only when properly trained, authorized, and permitted under applicable requirements and agency policy. Maintain a safe, clean, and supportive environment. Protect the individual's privacy and confidential information. Maintain appropriate professional boundaries. Complete required service documentation and progress notes accurately and on time. Document services provided, including progress toward individual goals and outcomes. Immediately report incidents, injuries, emergencies, suspected maltreatment, and other reportable events according to agency policy and applicable Minnesota requirements. Attend required staff meetings, supervision, training, and competency evaluations. Maintain all required certifications and training. Perform other duties related to the individual's authorized services as assigned by the supervisor.
