Job Description

Purpose of Job This position is responsible for providing personal care and homemaking services to home care and hospice patients under the supervision of an RN. Job Functions Documents the various aspects of care on appropriate forms. Communicates pertinent patient information and changes in schedule to RN/supervisor. Provides and assists personal cares, range of motion, exercises and other simple procedures according to the plan of care. Performs incidental housekeeping according to plan of care. Provides companionship/respite (Home Support and Hospice only). Provides assistance with medication (Home Support only). Performs home management tasks according to plan of care (Home Support only). Drives patients as requested and according to plan of care (Home Support only). Performs other duties as assigned. Minimum Education/Work Experience Certified Nursing Assistant Currently on the Minnesota Nursing Assistant Registry or willing to complete the CNA curriculum and/or competency evaluation to become current on the registry On Minnesota Nursing Assistant Registry (grandfathered prior to 2014) Successful completion of Ridgeview Home Health Aide competency test within 30 days of hire High school graduate or equivalent Knowledge/Skills/Abilities Certified Nursing Assistant and active on the Minnesota Nursing Assistant Registry (Grandfathered prior to 2014) Successful completion of Ridgeview Home Health Aide competency test within 30 days of hire High school graduate or equivalent License/Certifications Current Healthcare Provider CPR certification Valid MN driver's license and meets Ridgeview's insurability requirements Preferred Qualifications Previous home care, nursing home or hospice experience Link to Employee Health Physical Demands Compensation Many factors are taken into consideration when determining compensation such as: the requirements of the position, experience, education, knowledge, and skills along with location and internal equity. Pay listed does not include any shift, weekend, or other differentials. Employee works 4-5 days a week meeting FTE .6; Every third weekend. Most hours are between 8-5pm
