Job Summary We are seeking a compassionate and dedicated Personal Care Assistant to provide exceptional support and care to individuals in various settings, including assisted living facilities, nursing homes, and home health environments. This vital role involves assisting clients with daily activities, ensuring their safety, and promoting independence while maintaining dignity and respect. Responsibilities Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and toileting to promote comfort and independence Monitor patients' health by observing vital signs, patient behavior, and overall condition; report any changes promptly to healthcare professionals Support medication administration following prescribed guidelines while maintaining compliance with HIPAA regulations for patient privacy Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption as needed Use proper manual handling techniques and patient transfer methods, including Hoyer lifts and other equipment, to ensure safety during mobility assistance Maintain accurate care plans and documentation of patient progress, observations, and interventions in accordance with facility policies Provide personal transportation assistance for appointments or errands while ensuring client safety at all times Skills Proven experience working with individuals with disabilities, neurodevelopmental disorders, dementia, Alzheimer's disease, or memory care needs Strong knowledge of patient monitoring techniques, vital signs measurement, and first aid/CPR certification Familiarity with HIPAA regulations to ensure confidentiality of patient information Experience in home health or long-term care environments; group home or assisted living experience is highly desirable Ability to perform heavy lifting safely and execute manual handling tasks efficiently Excellent communication skills for effective client interaction and team collaboration Knowledge of care plans development and implementation tailored to individual needs Join us in delivering compassionate care that truly makes a difference!
