About the Role This position plays an important role in creating a culture of gentleness, dignity, respect, and inclusion . The QIDP oversees daily programming, supports individualized goals, coordinates with families and service providers, supervises staff, and helps ensure individuals experience meaningful, active, and person-centered days. The ideal candidate is organized, dependable, compassionate, and comfortable balancing direct support, staff leadership, documentation, program coordination, and administrative responsibilities. In this role, you will work directly with individuals served, develop and monitor support plans, coordinate services, and help individuals achieve their personal goals while maintaining their health, safety, and well-being. Pay $21.00 - $24.00 per hour, based on experience Schedule Monday-Friday, daytime hours. No evenings, weekends, or on-call responsibilities. What You'll Do Develop, implement, and monitor individualized care plans tailored to each individual's needs. Assist individuals with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, mobility support, meal preparation, and feeding assistance, including G-Tube feedings when required. Administer medications accurately while maintaining compliance with medical orders, agency policies, and documentation requirements. Conduct regular assessments of individuals' health status by monitoring vital signs, observing behavioral changes, and documenting findings appropriately. Collaborate with nurses, social workers, case managers, guardians, families, and interdisciplinary teams to coordinate comprehensive care and support. Manage behavioral challenges through positive behavior support techniques and conflict resolution strategies to maintain a safe environment. Maintain detailed documentation within Electronic Medical Record (EMR) systems, including care plans, incident reports, progress notes, and monthly summaries. Support individuals during community outings and transportation activities while adhering to all safety procedures. Maintain accurate, current, and timely daily and monthly documentation. Serve as an advocate for individuals by promoting independence, dignity, community inclusion, and person-centered services.

Required Qualifications:

Proven experience working with individuals with disabilities. (minimum of 2 years) Knowledge of medical terminology, medication administration procedures, and hospice or memory care practices. Familiarity with EMR systems for accurate documentation and case management. Ability to safely assist individuals with mobility needs, including transfers and the use of Hoyer lifts or similar equipment. Valid driver's license with a clean driving record for transportation responsibilities. Current First Aid and CPR certification required. Strong interpersonal skills to effectively communicate with individuals served, families, and team members while maintaining confidentiality and HIPAA compliance. Flexibility to adapt quickly to changing individual needs and work collaboratively within a team environment. Previous

experience in a day program setting preferred. Proven ability to supervise, coach, and support team of DSPs. Current driver's license and reliable transportation. Basic computer proficiency, including Microsoft Office Products. Strong organizational and time-management skills. Ability to complete required trainings and maintain applicable certifications.
