Role:

Coney Island Homecare is looking for a compassionate and dedicated Hospice Aide to join our caring team in Bozeman, MT. In this meaningful role, you'll provide essential hands-on support to patients and families during life's most important moments. Responsibilities:

Provide personal care assistance including bathing, grooming, dressing, and oral hygiene for hospice patients Assist with mobility, transfers, and repositioning to ensure patient comfort and safety Monitor and report changes in patient condition to the supervising nurse or care team Offer emotional support and companionship to patients and their families Assist with light housekeeping, meal preparation, and maintaining a clean, safe environment Document care activities accurately and in accordance with hospice care plans Collaborate with interdisciplinary hospice team to deliver coordinated, patient-centered end-of-life care

Requirements:

Current Certified Nursing Assistant (CNA) or Home Health Aide (HHA) certification preferred Prior experience in hospice, palliative care, or home health care strongly preferred Ability to provide compassionate, dignified care to terminally ill patients Strong communication skills and ability to work within a multidisciplinary care team Valid driver's license and reliable transportation for home visits in the Bozeman, MT area Ability to perform physical tasks including lifting, bending, and assisting with patient transfers Empathetic, patient, and emotionally resilient with a commitment to end-of-life care excellence
