Job Overview We are seeking a compassionate and dedicated Personal Care Assistant to join our team in providing exceptional support to individuals in various care settings. Duties Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and toileting to promote personal hygiene and comfort Monitor patient health by observing vital signs, reporting changes, and maintaining accurate care plans in compliance with HIPAA regulations Support medication administration under supervision or as trained, ensuring proper documentation and adherence to safety protocols Prepare nutritious meals tailored to individual dietary needs while maintaining cleanliness in the kitchen area Provide transportation assistance for appointments, errands, or community activities. Conduct patient observation to identify changes in condition or behavior, especially for those with Alzheimer's, dementia. Maintain a clean and safe environment by performing light cleaning duties and adhering to infection control standards Assist with mobility and transfer using proper patient transfer methods to ensure safety for both client and caregiver Support clients in participating in social activities and community engagement to promote mental well-being Document all care activities accurately and communicate effectively with healthcare professionals and family members regarding resident progress or concerns Experience Candidates should possess prior experience working in group homes, assisted living facilities, nursing homes, or home health environments. Experience working with individuals with disabilities—including developmental disabilities and intellectual disabilities—is highly valued. Knowledge of dementia care, memory care strategies, hospice care principles, and Alzheimer's support is preferred. Applicants should demonstrate proficiency in patient monitoring, vital signs measurement (such as blood pressure and pulse), medication administration. Certification in CPR and First Aid is required if you don't have we can provide. Familiarity with HIPAA compliance ensures confidentiality of client information. Prior roles involving social work or long-term caregiving enhance your suitability for this position. Experience supporting residents' rights while delivering compassionate personal care is essential for success in this role.

Pay:

$18.00 - $19.00 per hour Expected hours: 24.0 per week
