In this vital role, you will provide compassionate, personalized care that promotes independence, safety, and well-being. This paid position offers the opportunity to develop valuable skills in patient care, medication management, and community support while working in a rewarding and dynamic setting. Duties Assist individuals with activities of daily living (ADLs), including personal hygiene, dressing, grooming, and mobility support using manual handling techniques such as Hoyer lifts and patient transfer methods. Monitor patients' vital signs, observe behavioral changes, and report any concerns promptly to ensure safety and well-being. Support medication administration following care plans while maintaining compliance with HIPAA regulations to protect patient confidentiality. Prepare nutritious meals and assist with meal planning and preparation to promote healthy eating habits. Provide transportation assistance for appointments, outings, or community activities to foster independence and social engagement. Maintain a clean and safe environment by performing light cleaning duties and adhering to health and safety standards. Document care activities accurately in client records, including patient observations, progress notes, and incident reports to ensure continuity of care. Qualifications Prior experience working with individuals with developmental disabilities, autism spectrum disorder, dementia, or neurodevelopmental disorders is highly preferred. Knowledge of care techniques such as patient monitoring, Alzheimer's care, memory care, hospice care, and geriatrics enhances your ability to support diverse needs. Certification in CPR and First Aid is required; additional certifications such as medication administration or Hoyer lift training are advantageous. Strong communication skills to effectively interact with clients, families, team members, and healthcare professionals while respecting residents' rights. Ability to perform heavy lifting safely and assist with patient transfers using proper manual handling techniques. Experience working within home health settings or long-term care environments is beneficial; familiarity with HIPAA regulations ensures confidentiality at all times.
