Job Summary We are seeking a compassionate and dedicated Personal Care Assistant to provide exceptional support and care to individuals in various settings, including home health, assisted living, nursing homes, and community-based programs. This vital role involves assisting clients with daily activities, monitoring health status, and ensuring their safety and comfort. Responsibilities Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using manual handling techniques such as Hoyer lifts and patient transfer methods. Monitor patients' vital signs and observe changes in health status to report promptly to healthcare professionals. Prepare nutritious meals tailored to individual dietary needs and assist with meal planning and preparation. Support medication administration following care plans while adhering to HIPAA regulations for patient privacy and confidentiality. Maintain a clean and safe environment by performing light cleaning duties and ensuring infection control protocols are followed. Provide patient personal care assistance with compassion, ensuring dignity and respect at all times. Support clients during community outings or appointments using personal transportation assistance when needed. Document care activities accurately in compliance with healthcare standards and assist in updating individualized care plans regularly. Engage in patient observation for residents with dementia, Alzheimer's, autism, or other neurodevelopmental disorders, ensuring their safety through attentive supervision. Use specialized equipment such as Hoyer lifts or other assistive devices to facilitate safe patient transfers and prevent injury. Experience Prior experience working with individuals in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred. Demonstrated knowledge of dementia care, memory care, Alzheimer's care, hospice care, or geriatrics enhances your ability to support complex needs effectively. Experience working with individuals with developmental disabilities or neurodevelopmental disorders such as autism is advantageous. Familiarity with patient monitoring techniques including vital signs measurement and medication administration is essential. Proven ability to follow detailed care plans while maintaining compliance with HIPAA regulations is required. Hands-on experience with manual handling techniques like patient transfer methods or heavy lifting ensures safe support for clients. Strong communication skills for client interaction and team collaboration are vital to delivering consistent quality care.
