This vital role offers an opportunity to provide personalized, respectful, and professional care that promotes independence, safety, and well-being. As a caregiver, you will work closely with clients, families, and healthcare professionals to deliver comprehensive in-home assistance tailored to each person's unique needs. Responsibilities Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, mobility, and toileting to ensure comfort and dignity. Monitor patient health by observing vital signs, reporting changes, and maintaining accurate medical documentation using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems. Administer medications safely following prescribed instructions while adhering to HIPAA regulations to protect patient confidentiality. Support clients with specialized care needs including Alzheimer's care, dementia care, hospice support, catheter care, and memory care. Operate medical equipment such as Hoyer lifts for safe transfers and assist with complex procedures like wound care or medication administration. Prepare nutritious meals and assist with light housekeeping tasks including cleaning and laundry to maintain a safe and healthy environment. Document all patient interactions thoroughly in medical records and communicate effectively with healthcare teams regarding patient status or concerns. Qualifications Proven experience in home health care, assisted living, nursing homes, or long-term care environments. Knowledge of medical terminology, vital signs monitoring, medication administration, and infection control protocols. Certifications in CPR (Cardiopulmonary Resuscitation) and first aid are required; additional certifications such as hospice care or dementia training are a plus. Ability to work independently while following detailed care plans; experience with EMR/EHR systems is preferred. Strong observational skills for patient monitoring and the ability to assist with complex medical procedures like catheter care or hospice support. Compassionate attitude with excellent communication skills; experience working with individuals with developmental disabilities or disabilities is highly valued. Flexibility to provide live-in employment or scheduled visits based on client needs; valid driver's license preferred for community-based services.
