The Direct Support Professional will assist individuals with daily living activities, skill development, community participation, personal goals, and maintaining a safe and healthy living environment. The DSP will follow each individual's Person-Centered Plan or Individual Support Plan while promoting independence, dignity, choice, and community inclusion. Job Responsibilities Provide support according to the individual's Person-Centered Plan, Individual Support Plan, Behavioral Support Plan, and assigned goals. Assist individuals with activities of daily living, including bathing, grooming, dressing, toileting, eating, and mobility, when required. Teach and reinforce independent living skills, including meal preparation, housekeeping, laundry, budgeting, shopping, communication, and personal hygiene. Encourage individuals to make choices and participate in decisions regarding their daily routines and services. Support individuals with participating in recreational, social, educational, volunteer, employment-related, and community activities. Provide transportation to appointments and community activities when authorized. Implement approved behavioral support and de-escalation strategies. Observe and report changes in the individual's physical health, emotional well-being, behavior, or overall functioning. Provide medication reminders or assistance with self-administration when properly trained and authorized. Maintain a clean, safe, and supportive environment. Complete accurate and timely service notes, progress notes, incident reports, electronic visit verification, and other required documentation. Protect the individual's privacy, confidentiality, dignity, and rights. Immediately report suspected abuse, neglect, exploitation, injuries, medical emergencies, behavioral crises, medication concerns, or other safety issues. Communicate professionally with individuals, families, guardians, supervisors, and other members of the support team. Attend required meetings, orientation sessions, training, and supervision. Follow all Serenity at Heart policies, procedures, safety standards, and professional expectations. Minimum Qualifications Must be at least 18 years of age. High school diploma or GED preferred. Experience working with individuals with intellectual and developmental disabilities or behavioral health needs is preferred but not required. Must be compassionate, patient, reliable, and able to maintain professional boundaries. Must be able to read, write, communicate effectively, and follow written service plans. Must successfully complete all required background checks and registry screenings. Must obtain or maintain current CPR and First Aid certification. Must have reliable transportation to assigned service locations. A valid driver's license and automobile insurance may be required for assignments involving transportation. Must be willing to complete all required agency and individual-specific training. Must be able to use a smartphone or electronic system for scheduling, documentation, and electronic visit verification. Physical Requirements The Direct Support Professional must be able to perform the essential duties of the position with or without reasonable accommodation. Duties may require standing, walking, bending, reaching, assisting with mobility, and supporting individuals with transfers or personal care. Physical requirements may vary depending on the needs of the assigned individual.

Pay:

$17.00 - $19.00 per hour
