Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using proper patient transfer methods such as Hoyer lifts and manual handling techniques. Monitor patients' health by observing vital signs, tracking changes, and reporting concerns to healthcare professionals promptly. Support medication administration following HIPAA guidelines and care plans to ensure safe and accurate delivery of treatments. Prepare nutritious meals tailored to dietary needs while maintaining cleanliness and hygiene standards in the client's environment. Provide transportation assistance for appointments, errands, or community activities to foster independence and social engagement. Maintain detailed documentation of care activities, patient observations, and adherence to resident's rights policies. Engage in patient monitoring during hospice care or memory care situations, including Alzheimer's disease, dementia care, or neurodevelopmental disorders. Experience Prior experience working with individuals with disabilities, developmental or intellectual disabilities support, or senior care is highly valued. Demonstrated knowledge of patient care techniques such as first aid, CPR, and emergency response procedures. Experience working in group homes, nursing homes, assisted living facilities, or home health environments is preferred. Familiarity with care plans development and medication administration processes ensures quality support for clients. Ability to perform heavy lifting safely and assist with patient transfer methods like Hoyer lifts or patient personal transportation assistance. Proven ability to communicate effectively with clients and team members while respecting residents' rights and maintaining confidentiality under HIPAA regulations.
