Responsibilities Assist clients with activities of daily living (ADLs), including personal hygiene, dressing, grooming, and mobility support Monitor patients' health by taking vital signs and observing for any changes or concerns Support clients with medication administration following prescribed care plans and HIPAA regulations to ensure privacy and safety Prepare nutritious meals and assist with meal planning tailored to individual dietary needs Facilitate patient transfer methods using equipment such as Hoyer lifts and ensure safe manual handling practices during patient transfers Maintain accurate documentation of care provided, patient observations, and adherence to resident's rights and safety protocols Support clients during community outings or appointments using personal transportation assistance while ensuring their safety Qualifications 1. Personal Care and Daily LivingAssist residents with activities of daily living (ADLs), including bathing, dressing, grooming, and personal hygiene.

Support proper toileting and mobility needs using safe transfer techniques (e.g., gait belts or lifts).Assist with feeding and meal preparation according to specific dietary guidelines. 2. Health and Medication ManagementAdminister or assist with self-administration of medications following certified training.

Monitor vital health signs, manage chronic conditions, and report any changes in a client's physical or mental condition.

Accompany residents to scheduled medical, dental, or therapy appointments. 3. Home ManagementPerform household duties, including light housekeeping, laundry, and general cleaning.

Plan weekly menus, grocery shop, and maintain an organized, safe living space. 4. Skill Building and Community IntegrationTeach and reinforce independent living skills, such as money management, cooking, and self-help.

Plan and support participation in social activities, hobbies, and local community outings.

Act as an advocate to protect residents' rights and support positive peer and community relationships. 5. Documentation and ComplianceMaintain accurate daily logs, behavioral data, and service notes using electronic health record (EHR) systems.

Follow individual support and behavior intervention plans.

Maintain mandatory state regulatory certifications (such as CPR, First Aid, and medication administration).

Education:

High school diploma or GED equivalent.

Age:

At least 18 years old (often 21+ depending on state driving regulations).

Driver's License:

Valid state driver's license with a reliable driving record and auto insurance (for client transport).

Background Check:

Ability to pass criminal record checks and healthcare registries.

Physical Demands:

Ability to bend, stoop, walk, and lift up to 40 pounds independently.

Soft Skills:

High level of patience, empathy, strong communication skills, and a positive, compassionate attitude.
