Promote independence, dignity, self-determination, and community inclusion. Implement individualized service plans, teaching strategies, and positive behavior support plans. Assist participants with personal development goals, life skills training, communication, socialization, and community engagement. Create and facilitate engaging activities that support learning, cognitive development, and personal growth. Support individuals during community outings, recreational activities, educational workshops, and volunteer opportunities. Encourage positive relationships and meaningful participation within the community. Monitor participant progress and communicate concerns or changes to program leadership. Complete documentation, data collection, assessments, and required reports accurately and timely. Assist with activities of daily living, meals, and personal care as needed. Maintain a safe, healthy, and supportive environment. Transport individuals safely using company vehicles. Maintain confidentiality and promptly report any concerns regarding participant health, safety, abuse, neglect, or exploitation. Work collaboratively with Site Managers, QDDPs, team members, families, and other stakeholders.
