The ideal candidate will possess a strong background in patient care, including experience with geriatric and disabled populations, and demonstrate a commitment to enhancing the quality of life for those they serve. This role offers weekly pay and the opportunity to make a meaningful difference in the lives of individuals needing personalized support within their homes. Duties Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support using patient transfer methods and equipment such as Hoyer lifts. Monitor and observe patient conditions, recording vital signs accurately and reporting any changes to healthcare professionals. Administer medications following medical instructions, ensuring compliance with home healthcare regulations and maintaining detailed medical documentation. Support patients with memory care needs, including Alzheimer's disease and dementia, providing compassionate end-of-life care when necessary. Maintain patient confidentiality in accordance with HIPAA regulations while managing medical records and documentation securely. Prepare nutritious meals tailored to individual dietary requirements, assisting with cooking and ensuring proper nutrition. Perform light housekeeping tasks such as cleaning, laundry, and maintaining a safe environment to promote overall well-being. Collaborate effectively with healthcare teams using EMR (Electronic Medical Records) or EHR (Electronic Health Records) systems to ensure seamless communication and continuity of care. Experience Prior experience working as a Home Health Aide or in assisted living or nursing home settings is highly desirable. Demonstrated proficiency in patient observation, medical terminology, medication administration, and vital signs monitoring. Experience working with individuals with developmental disabilities, geriatrics, or those requiring hospice or palliative care is preferred. Familiarity with home health care assistance, home & community care practices, and long-term care environments. Knowledge of home health agency operations, medical documentation standards, and compliance with home health regulations. Hands-on experience using patient transfer methods such as Hoyer lifts or other mobility aids safely. Ability to perform heavy lifting when necessary while maintaining safety standards for both patients and caregivers.
