Job Summary We are seeking a dedicated and compassionate Home Health Aide to provide exceptional care and support to clients in their homes. The ideal candidate will possess a strong background in home healthcare, with experience working with diverse patient populations, including seniors, individuals with disabilities, and those requiring specialized care such as dementia or hospice. As a Home Health Aide, you will play a vital role in assisting clients with daily activities, ensuring their safety, comfort, and well-being while adhering to all healthcare regulations and maintaining strict confidentiality. Responsibilities Assist patients with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support using patient transfer methods and equipment such as Hoyer lifts. Monitor and observe patients' conditions, including vital signs, and report any changes to healthcare professionals promptly. Administer medications as prescribed, ensuring proper documentation in electronic health record (EHR) systems while maintaining compliance with HIPAA regulations. Provide specialized care for patients with Alzheimer's disease, dementia, or other memory impairments, including memory care techniques and behavioral management. Support patients during end-of-life care or hospice situations with compassion and dignity while respecting their wishes and comfort. Perform light housekeeping tasks such as cleaning, laundry, and meal preparation to promote a safe and healthy environment. Maintain accurate medical records and documentation of patient care activities in accordance with home healthcare regulations. Communicate effectively with clients and their families to ensure personalized care plans are followed and needs are met consistently. Experience Prior experience working as a Home Health Aide or in a similar home healthcare setting is strongly preferred. Demonstrated proficiency in patient observation, medical terminology, medication administration, and vital signs monitoring. Experience assisting with ADLs, including catheter care and other specialized medical procedures. Familiarity with EMR (Electronic Medical Record) systems and home health agency operations. Knowledge of geriatric care, hospice support, dementia care, and working with individuals with developmental disabilities or disabilities is highly desirable. Certification in first aid and CPR is required; additional training in basic life support (BLS) is advantageous. Ability to lift heavy objects safely and perform patient transfers using appropriate techniques and equipment such as Hoyer lifts.
