Working closely with the RN Care Coordinator and other members of the Williams Adult Day Center team, this position helps ensure participants are safe, comfortable, engaged, and supported throughout the day. This is a PRN (as-needed) position . Hours vary based on program needs and are not guaranteed. A PRN team member may work several days one week, fewer days another week, or may not be scheduled during some weeks. What You'll Do Provide personal care according to each participant's plan of care and direction from the RN Care Coordinator or designee. Assist participants with transfers, ambulation, activities, and programs. Observe participants and promptly report unusual occurrences or changes in condition. Assist with obtaining vital signs and weights as directed. Maintain accurate and timely participant records. Follow established procedures for participant medical emergencies. Build positive, respectful relationships with participants, families, and team members. Participate in required meetings, in-services, and training. What We're Looking For High school diploma or equivalent. Certified Nursing Assistant (CNA) or Personal Care Assistant (PCA) certification or at least one year of related experience in geriatrics or caring for individuals with dementia. Current CPR and First Aid training. Experience working with older adults preferred. Good judgment, communication, interpersonal, and recordkeeping skills. Ability to assist participants with transfers and mobility. Ability to lift 25 pounds or more and stand and walk for most of the workday.
