Monitor patients' health by observing vital signs, reporting changes, and maintaining accurate care plans in compliance with HIPAA regulations. Support medication administration under supervision or as directed by healthcare professionals, ensuring proper documentation and adherence to safety protocols. Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption when necessary. Provide companionship and emotional support to promote mental well-being, especially for clients with dementia, Alzheimer's disease, or other neurodevelopmental disorders. Maintain a clean and safe environment through regular cleaning duties and adhering to infection control standards. Assist with transportation needs for appointments or community outings while ensuring client safety at all times. Skills Proven experience working in group homes, assisted living facilities, nursing homes, or home health environments. Strong patient observation skills with the ability to recognize changes in condition or behavior promptly. Knowledge of dementia care, Alzheimer's care, memory care, hospice care, and resident's rights. Ability to perform patient transfers using Hoyer lifts and other manual handling equipment safely. Familiarity with medication administration procedures and vital sign monitoring. Excellent communication skills for effective client interaction and documentation of care activities. Certification in CPR and First Aid is preferred; ongoing training in HIPAA compliance is essential. Experience working with individuals with autism, developmental disabilities, intellectual disabilities support needs or neurodevelopmental disorders is highly valued.
