Education:

HS Diploma or GED required

Eligibility/Experience Requirements :

Valid Driver's license One year's experience I/DD (preferred) Assist consumers, as needed, in skill development, activities of daily living, personal care, domestic living, recreation and socialization Ensure assigned clients' nutritional needs; assisting with feeding as necessary Assist consumers in nutritional planning, grocery shopping and preparation of all meals. Collaborate on the development/implementation of consumers' Individual Service plans (ISP). Train and support consumers in all areas of their ISP. Assist with medication administration. Train and support consumers in the community including: social, recreational, cultural, religion and education networks.
